LICENSING INFORMATION REQUIREMENTS AND UPDATES
New Forms and Requirements

Apologies
to all for the temporary composition of this page in the context of
the website. We have a good deal of new information to offer, and
are working on a website re-design. In the interim, some new data
is necessary for the consideration of applicants, hiring managers,
and/or facility owners
- All
new applicants for licensure must complete a criminal records
background check. There is a very specific protocol that must
be followed. Please proceed as indicated in the “Criminal
Record Checks Now Required” document linked
here. Please report here any problems or circumstances
you encounter that differ from the protocol as described.
- While
working on application re-design, we have developed new application
checklists for each license type we offer. Please download, print
out and use the checklists as linked below to accompany each new
application and to assure you are meeting requirements:
